Eligibility : Passed Intermediate, 10+2 or equivalent examination in English, Physics, Chemistry and Biology (Zoology + Botany) taken together; (i) For MBBS/BDS/BAMS – a minimum of 50% for General, OBC and respective PC candidates and 40% aggregate marks for SC/ST and respective PC candidates; (ii) For B.Pharm. – a minimum of 60% marks for General, OBC and respective PC candidates and 55% aggregate marks for SC/ST and respective PC candidates. Candidates appearing in qualifying examination may also apply.
Application procedure : BY CASH : on payment of Rs. 1100/- (Rs. 700/- for SC/ST candidates) from the following branches of Bank of Baroda. Application Form can be obtained from Bank of Baroda, BHU Branch, S.S. Hospital only, upto Friday, 05.03.2010 till 4:00 PM..
BY POSTAL CHANNEL : From the Office of the Director (PMT-Cell), Institute of Medical Sciences, Banaras Hindu University, Varanasi-221 005 by sending a written request through Registered Post along with Crossed Demand Draft/Bankers Cheque (MICR only) of Rs. 1200/- (Rs. 800/- for SC/STcandidates) of a Nationalized Bank, drawn in favour of “The Director, Institute of Medical Sciences, BHU” payable at Varanasi. Application form will be sent by Speed / Registered Post. However, Institute will not be responsible for any loss or postal delay.
The request for application form must include the name and address of the candidate, and full particulars of the enclosed DD/BC (write candidate’s name and address on the back of DD). Two self-addressed, preferably typed, white paper slips of 10 cm X 6 cm size MUST also accompany it.
The ENVELOP containing requisition for PMT application form should be super scribed “ Request for Application for PMT-2010, BHU”. Last date of requisition for PMT form by post is Friday, the 19th February 2010.
Incomplete/non-cashable Draft (under amount/ unsigned/ without Bank Code No. & Seal) shall not be accepted.
NOTE : (a) Separate Bank Draft will be required for each application form. (b) M.O./ I.P.O./Cheques shall NOT be accepted under any circumstances. Application Forms from the Office of the Director (PMT-Cell), Institute ofMedical Sciences, can be obtained by POST only and not in person. In case forms are not available at Bank of Baroda Branches, the same can be obtained by post from Director’s Office upto 19th February, 2010.
